How do I request advice?

Prior to disclosing a wrongdoing, it is recommended that you seek advice. You can seek advice from a supervisor, the designated officer of your public entity, if one exists, or the Office of the Public Interest Disclosure Commissioner (OPIDC). This service is free and confidential. Please contact us for a copy of the Request for Advice Form.

How do I disclose a wrongdoing?

Disclosures of wrongdoing must be in writing and to fulfill that requirement, the Office of the Public Interest Disclosure Commissioner (OPIDC) has developed the Disclosure of Wrongdoing Form. Please contact us for a copy of this form, then submit it using secure file transfer or deliver in person to the OPIDC; do not email the form as this is not a secure method of communication. How do I make a reprisal complaint?

Complaints of reprisal must be in writing and to fulfill that requirement, the OIPDC has developed the Reprisal Complaint Form.
